Mandatory Training Notice


A mandatory one-time online training is required for all new and existing Shop@UW Primary Account Contacts. The Primary Contact is responsible for purchases made with the account and may be contacted with questions related to account activity.

The following request types will not be processed until it has been confirmed that the Primary Account Contact has completed the training.

  • New Account Requests
  • Account Reactivation
  • Primary Contact Updates

Training should take about 15 minutes to complete and you will need your Employee ID to complete the quiz.  Access the training and additional information about it here.

New Users and Accounts


New users from UW campuses will need to set up an account to use Shop@UW.

Please click the link for your campus from the list in the “Account Setup or Revision Request Forms” section on the right hand side of this webpage. Before you start shopping, take a look at our training resources page for training videos and guides.

Existing Users and Accounts


Please click the link for the Account Set-Up or Revision Request form for your campus from the list found in the box at the right.

If you are using an MSWord format form to revise an account, check the “Existing Account” option box and enter the existing MD account number in the field at the top of the form.

Account Setup or Revision Request Forms

MSWord format: Click the link to download the form. Fill out the document, print, obtain signatures, and submit as directed at the top of the form.

Online form submission: Click the link to access the form. Once you submit it, the request will be routed through an automated approval process.
